It does not evaluate clinical care, legal compliance, staff merit, contract entitlement, or the correctness of a final payer decision.

Methodology: Freeze the population, source report, filters, cutoff, timezone, duplicate rule, and exclusions before outcomes are reviewed. Preserve adjustment group and reason codes, remark codes, allowed amounts, patient-responsibility fields, claim lineage, and local posting state before assigning a descriptive category.
