Usage Billing Reconciliation Research: Matching Activity to Billable Records

Research date: 2026-08-07. Scope: operational research for a billing owner designing a Philippines-based support workflow; this is not legal, coding, or clinical advice.

Usage billing requires a clear bridge from activity to billable quantity. The queue should identify the source event or report, period, account, rule or contract reference, units, exclusions, draft line, and reviewer.

Method: I compared the current CMS billing and coding references with HHS privacy and security requirements, NIST control guidance, ONC safety material, and AHRQ communication guidance. The result is a control-oriented checklist: preserve the source record, separate preparation from approval, and make exceptions visible.

Reconcile the activity report to the account and billing period, then check duplicates, missing events, cutoff errors, unit conversions, minimums, caps, and manual overrides. Preserve the report version and note any assumption that requires owner confirmation.

A practical first-week test is to give the specialist a small mixed queue, require a source reference on every completed item, and review all exceptions before increasing access. Measure completeness, correct routing, aging, and rework rather than raw item volume.

A specialist can perform the documented match and prepare an exception packet. The owner approves contract interpretation, overrides, disputed usage, credits, and release. A missing or contradictory activity source remains open until resolved.
