Billing SOP Quality Metrics Research: Measuring Work Without Rewarding Guesswork

A small scorecard can show queue health while keeping accuracy, evidence, and escalation ahead of raw speed.

A billing support scorecard should measure whether work is complete, traceable, and routed correctly. Counting closed items alone can reward shortcuts, especially when a specialist closes uncertain records instead of exposing them for review.

Method: I compared the current CMS billing and coding references with HHS privacy and security requirements, NIST control guidance, ONC safety material, and AHRQ communication guidance. The result is a control-oriented checklist: preserve the source record, separate preparation from approval, and make exceptions visible.

Track a small set of measures tied to the workflow: records completed with a source reference, exceptions routed to the right owner, aged items by status, returned items after review, unresolved reconciliation differences, and access or privacy incidents. Define the sample and review period before comparing results. Do not turn national CMS figures into a local performance target.

A practical first-week test is to give the specialist a small mixed queue, require a source reference on every completed item, and review all exceptions before increasing access. Measure completeness, correct routing, aging, and rework rather than raw item volume.

The specialist can maintain the queue and provide the evidence behind each measure. The owner decides which measures matter, reviews a sample of closed work, and changes the SOP when errors repeat. Use the scorecard to improve instructions and routing, not to pressure staff into making decisions outside their role.
