Medical Billing Remittance Review: A Research-Based Reconciliation Workflow
Research-backed controls for tying remittance advice to claim records, deposits, adjustments, and unresolved balances.

Remittance review is strongest when each posting decision can be traced from the payer explanation to the claim, account, deposit, and adjustment rule. The goal is not merely to make a balance change; it is to make the change explainable to the next reviewer.
Method: I compared the current CMS billing and coding references with HHS privacy and security requirements, NIST control guidance, ONC safety material, and AHRQ communication guidance. The result is a control-oriented checklist: preserve the source record, separate preparation from approval, and make exceptions visible.
Create one review row per transaction or defined batch with payer, trace or check reference, service or claim reference, billed amount, paid amount, contractual or other adjustment category, patient responsibility where applicable, posting status, and exception reason. Reconcile totals to the deposit or electronic remittance batch before releasing the work.
A practical first-week test is to give the specialist a small mixed queue, require a source reference on every completed item, and review all exceptions before increasing access. Measure completeness, correct routing, aging, and rework rather than raw item volume.
The specialist can prepare and match records, but the owner approves unusual adjustments, refunds, transfers, and balance corrections. Preserve the original remittance and a dated note explaining any unresolved difference.
