Refund Request Controls: Research on Billing Evidence and Approval
Refund work should be a review packet, not an unchecked balance adjustment.

A refund request joins payment evidence, account history, policy, and destination details. Because a mistaken refund can create a second reconciliation problem, the preparation workflow should make the proposed action easy to challenge before it is approved.
Method: I compared the current CMS billing and coding references with HHS privacy and security requirements, NIST control guidance, ONC safety material, and AHRQ communication guidance. The result is a control-oriented checklist: preserve the source record, separate preparation from approval, and make exceptions visible.
Capture the request source, account reference, payment trace, amount, date, original posting, related claim or invoice, prior refund or reversal activity, duplicate check, destination verification, and the exact policy or owner rule being applied. Keep preparation and release as separate permissions.
A practical first-week test is to give the specialist a small mixed queue, require a source reference on every completed item, and review all exceptions before increasing access. Measure completeness, correct routing, aging, and rework rather than raw item volume.
A specialist can assemble the packet and flag missing or conflicting evidence. The named owner approves the refund, reversal, transfer, amount, and destination. If the source payment or requester identity cannot be established, stop and escalate rather than filling the gap with an assumption.
