Billing Dispute Intake Research: Separating Questions from Adjustments
A dispute intake queue gives each question a source, requested outcome, deadline, and decision path before any balance changes.

Dispute intake should capture the original question before the team proposes a result. Record the invoice or claim reference, disputed line, date received, requested outcome, supporting records, response date, status, and decision owner.
Method: I compared the current CMS billing and coding references with HHS privacy and security requirements, NIST control guidance, ONC safety material, and AHRQ communication guidance. The result is a control-oriented checklist: preserve the source record, separate preparation from approval, and make exceptions visible.
Preserve the original message and compare it with the invoice, payment, service, contract, or claim records allowed by policy. Classify whether the item needs explanation, correction, credit, refund, coding review, or escalation. Keep the intake record separate from the approved adjustment.
A practical first-week test is to give the specialist a small mixed queue, require a source reference on every completed item, and review all exceptions before increasing access. Measure completeness, correct routing, aging, and rework rather than raw item volume.
The specialist can build the evidence packet and draft a factual chronology. The owner decides the response, adjustment, credit, refund, appeal, or escalation. Route privacy concerns, legal threats, and contradictory sources before replying.
