Billing Data Quality Review: Research on Exception-First Sampling

A small, mixed sample can reveal more operational risk than a volume-only review.

Data quality review should test whether the record is complete, consistent, traceable, and safe to act on. Sampling only easy items can make a queue appear healthy while leaving the hardest exceptions invisible.

Method: I compared the current CMS billing and coding references with HHS privacy and security requirements, NIST control guidance, ONC safety material, and AHRQ communication guidance. The result is a control-oriented checklist: preserve the source record, separate preparation from approval, and make exceptions visible.

Define a sample that includes routine matches, missing fields, duplicates, conflicting dates, unapplied amounts, rejected claims, and records with restricted information. For each item, compare the system record to the source, record the defect category and materiality, assign an owner, and track whether the fix changes the underlying workflow.

A practical first-week test is to give the specialist a small mixed queue, require a source reference on every completed item, and review all exceptions before increasing access. Measure completeness, correct routing, aging, and rework rather than raw item volume.

A Philippines-based reviewer can perform the documented comparison within approved access. The billing owner decides whether a defect requires correction, retraining, policy change, coding review, or broader sampling. Report defect patterns separately from individual productivity.
