Credit Memo Governance Research: Evidence Before Balance Changes
Credit memo preparation should expose the original charge, reason, authority, and downstream reconciliation before approval.

A credit memo changes a customer or account balance, so preparation should begin with the original invoice or charge and the request that explains the reason. The queue should name the affected line, amount, date, source, requestor, and approval owner.
Method: I compared the current CMS billing and coding references with HHS privacy and security requirements, NIST control guidance, ONC safety material, and AHRQ communication guidance. The result is a control-oriented checklist: preserve the source record, separate preparation from approval, and make exceptions visible.
Check the original billing record, prior credits, payment status, dispute or service record, applicable policy, and destination ledger. Separate a request for explanation from an approved credit. Flag duplicate requests, unsupported reasons, tax implications, and credits that would create an unusual balance.
A practical first-week test is to give the specialist a small mixed queue, require a source reference on every completed item, and review all exceptions before increasing access. Measure completeness, correct routing, aging, and rework rather than raw item volume.
The specialist can assemble evidence and draft the memo. The owner approves the amount, reason, tax treatment, customer response, and posting. Never release a credit because a request appears reasonable without the required source and approval.
