Charge Entry Review: Evidence-Led Research for Billing Operations
A charge-entry review should connect the encounter source to coding, payer edits, and owner-approved exceptions.

Charge entry is a controlled translation from an encounter or service record into a billing record. The review question is whether the entered record is supported by the permitted source and whether any coding or edit issue has been routed to the right expert.
Method: I compared the current CMS billing and coding references with HHS privacy and security requirements, NIST control guidance, ONC safety material, and AHRQ communication guidance. The result is a control-oriented checklist: preserve the source record, separate preparation from approval, and make exceptions visible.
Keep the source encounter reference, service date, provider or location context, entered codes and modifiers, required documentation status, edit result, and reviewer note together. Use current CMS and payer guidance as references, but do not let a queue worker resolve clinical documentation or coding ambiguity by inference.
A practical first-week test is to give the specialist a small mixed queue, require a source reference on every completed item, and review all exceptions before increasing access. Measure completeness, correct routing, aging, and rework rather than raw item volume.
A specialist can identify missing fields, duplicate-looking charges, and failed edits. A qualified coder or billing owner makes clinical or coding judgments and approves corrections. Record the final decision and its source so later rework does not depend on memory.
