Billing Document Access Denial Research: Separating Missing Proof From Permission Limits

How to distinguish an absent document from a document that exists but is outside the assigned access boundary.

Scope and method: for each sampled request, record the systems and queues searched, date filters, identifiers used, returned access response, known document reference, request path, stopping point, and owner responsible for granting or locating approved evidence. Keep “not found,” “not searched,” and “not permitted” as separate states.

The evidence shows why access status matters. A search that returns no document is different from a search that cannot be completed under least-privilege permissions. Treating both as missing proof can lead to an unsupported billing conclusion or pressure a worker to bypass controls.
