Billing Approval Matrix Research: Assigning Decisions to the Right Owner
An approval matrix clarifies which billing actions support staff can prepare, which owners must review, and where work must stop.

A billing approval matrix turns vague authority into a visible queue rule. For each action, list the preparation role, required source, review owner, approval evidence, system permission, and escalation route.
Method: I compared the current CMS billing and coding references with HHS privacy and security requirements, NIST control guidance, ONC safety material, and AHRQ communication guidance. The result is a control-oriented checklist: preserve the source record, separate preparation from approval, and make exceptions visible.
Map routine preparation separately from code changes, credits, refunds, write-offs, contract interpretation, patient or customer conclusions, and access changes. Check that the permissions match the written matrix, that substitute approvers are named, and that every exception has a dated owner.
A practical first-week test is to give the specialist a small mixed queue, require a source reference on every completed item, and review all exceptions before increasing access. Measure completeness, correct routing, aging, and rework rather than raw item volume.
A specialist can prepare work inside the assigned permission boundary and record the source. The named owner approves decisions reserved to that role and records the reason. If the matrix does not answer a case, pause the action and escalate instead of relying on informal precedent.
