Start with the awkward fact, not the dashboard label. This routine covers a mismatch among service location, billing location, enrollment record, and claim fields. Give the item a stable locator, record when it entered the queue, and name the credentialing, coding, or billing owner. A colleague taking over tomorrow should see the same question you see today.

Open the evidence side by side: encounter source, claim version, location identifiers, dated enrollment evidence, payer edit, and interface mapping. Preserve source wording and identifiers in the approved billing system. Record event time separately from entry, retrieval, and review time; those timestamps answer different questions.

The practical danger is that similar location names can conceal different identifiers, effective periods, or billing relationships. Mark every value as observed, calculated, owner-interpreted, or unknown. When two sources disagree, keep both.
